Nursing Diagnosis for Common Diseases on the Organ Trail

Not everthing that starts in Missouri has to end with Dysentery....

Remember the days, long ago, before nursing school, when we could play really awesome games like The Oregon Trail??


Well, if you ever wondered what those diseases were that your sister died from after fording the river get ready to be amazed.... or it's Monday and you don't want to be productive. Whatever.

Typhoid Fever

We didn't know the importance of purifying water, yet. This came from contaminated water sources. Salmonella typhi was the culprit. You would have a high fever, weakness, and a loss of appetite. 

It's still an issue in undeveloped nations. In 2013 it accounted for 161,000 deaths worldwide.

Nursing Diagnosis:
  • Activity Intolerance- Related to mandatory bed rest
  • Risk for Fluid Volume Deficit- Related to nausea, vomiting, decreased fluid intake, or excessive spending (diarrhea, sweating)
  • Imbalanced Nutrition, Less Than Body Requirements- Related to Nausea, Vomiting, or Anorexia
  • Acute Pain- Related to inflammation of the small intestine
  • Knowledge Deficit


Dysentery was spread in contaminated water and foods. It thrives in hot humid conditions. It's also known as Shigellosis. 

It attacks and kills the cells in the intestines and replicates itself. It lives in the colon and produces loose, bloody stools. It can be accompanied with abdominal pain, and fever. 

Dysentery is still a major disease in under-developed nations. It's also evolving to be more resistant to antibiotic treatment. The WHO estimates the number of those infected world-wide to be around 80 million and 700,000 of those infected die. 

Nursing Diagnosis:

  • Imbalanced Nutrition: Less Than Body Requirements- Related to diarrhea, or decreased intake
  • Fluid and Electrolyte Imbalances- Related to diarrhea
  • Ineffective Thermoregulation- Related to the process of infection
  • Impaired Skin Integrity- Related to diarrhea
  • Acute Pain- Related to inflammation of the intestines


Cholera is an infection of the small intestine from the bacterium Vibrio cholerae. Cholera was a major killer on the Oregon Trail. Also, it cost me a few good men. However, that elongated my supplies. Eh. You win some, you lose some. 

This, too, was from contaminated water and food sources. It causes the intestines to be flooded with water, so you have lots of diarrhea. This leads to severe dehydration, and if you are set out to keep your oxen on a strenuous pace and not paying attention to your sick comrade, he/she will most likely die.

There's limited vaccine's for Cholera with so-so efficacy. It's still a major disease of undeveloped countries. It effects 3-5 million people world wide. 115,000 die. Treatment includes iv fluid therapy and antibiotics.

Nursing Diagnosis

  • Deficient Fluid Volume- Related to excessive fluid loss (diarrhea, vomiting)
  • Imbalanced Nutrition: Less Than Body Requirements- Related to loss of fluids or deficient intake
  • Impaired Skin Integrity- Related to diarrhea
  • Altered Bowel Elimination- Related to increased peristalsis
  • Self Care Deficit- Related to weakness

Bottom line:

  • Nurses have a sick twisted sense of humor.
  • My childhood was awesome.
  • We need to work on getting everyone in the world safe, clean drinking water.
  • This post wasn't a complete waste of your time if you ever plan on doing a medical mission you might actually use this information.
